Multi-Purpose Permission Form

Hopefully your child will have brought home a Multi-Purpose Permission letter.  This letter contains details of activities that your child may take part in while at school that we need your permission for.  The attached consent form also has space for you to provide any details of health conditions or allergies that we should be aware of. 

 

Please complete the form and return to school as soon as possible.

 

If you have not received this letter can you please request one from the school office.

 

The information on the form will be used throughout your child's time at Blakenall Heath but consent can be changed at any time.
